Please allow two hours for an examination if you are a new patient. Bring your medication list and insurance card to your examination.

To facilitate your visit you can download and complete a Patient History Form and bring it with you to your examination. You can also read our Notice of Privacy Practices online.
 
 

EYE EMERGENCIES


Dr. Clark is available after hours to see her current patients. You can reach her at the number listed on the office answering machine. If you cannot reach her immediately please go to your nearest hospital emergency room or call your primary care physician.

Discount for Cash or Check - If we will not be billing insurance we offer a generous pay-at-the-time of-service discount for your glasses and contact lenses. Your balance must be paid at the time of service to qualify for this discount.
Credit Card - We accept Visa, MasterCard, DIscover and American Express
Financing - We work with Care Credit financial corporation to offer 6 or 12 month interest-free payment periods. If you choose to extend your payment period beyond the initial 12 months significant interest fees will apply.
Health Savings Account or Medical Savings Account - Present your debit card at the time of service or we can give you a receipt for you to be reimbursed.

Because our office provides both routine and medical eye care, Clark Eye Care accepts a number of insurance plans to help cover the cost, depending on your individual needs. Regular Health Insurance plans protect you against financial losses due to eye injuries or eye disease. Vision Insurance, on the other hand, is a wellness benefit that provides routine vision examinations, and helps cover the cost of glasses and contact lenses.
We are happy to bill your medical insurance provider for you if we are in network. We can get an estimate of your benefits so you'll have a better idea of what your insurance may cover We can also provide you the necessary information so that you can bill any out-of-network coverage you may have. If you have vision care coverage we will obtain authorization and advise you of your vision care benefits either before or at the time of our appointment.
When we quote you benefits, this is only an estimate of your coverage and not a guarantee of payment. Consult the benefits information that you received from your employer for more detailed information. You are responsible for all charges.
